The Tribal Administration has issued the official Notice of Per Capita Distribution. The full packet — including the Per Capita Survey, Mail Authorization Form, and W-9 — is available at the Tribal Administration Office or online.
Key dates
- Distribution opens: Monday, August 3, 2026, at 9:00 AM
- Last day to submit documents: Wednesday, September 2, 2026, at 5:00 PM — the Tribe is not responsible for late mail
What you must submit (all three)
- Tribal Per Capita Survey — filled out completely and legibly, including your enrollment number, signed and notarized. Notary services are available at the Tribal Administration Office.
- W-9 form, signed
- A copy of your driver's license, state ID, or Tribal ID card
How to submit
- In person at the Tribal Administration Office, 67 NW Maple Street, Tuba City, AZ
- Or scan and email your application to dist@sanjuanpaiute-nsn.gov — photos taken by phone or texted photos will not be accepted; documents must be scanned
- Applications must be submitted within the 30-day period
Receiving your check
- Checks are released only to the enrolled member themselves — no pick-up on behalf of others
- If you want your check mailed, complete the Mail Authorization Form in the packet; mailed checks go by certified mail to the address you provide
Please remember this is a community event. All members are asked to treat one another and tribal employees with courtesy and respect; the full conduct notice is in the packet. Members are responsible for any income taxes owed on their payment, per Tribal Council Resolution No. 2026-033.
