Covid 19 Office Visit Procedures

Before you visit the office.

if you are experiencing FLU LIKE SYMPTOMS, If you tested POSITIVE for C19 or have been in CONTACT with someone diagnosed with Covid-19, feeling SICK, or Quarantined, please DO NOT come to the office. Instead reach out thru Telephone, email or text.

For Distribution please, call the office and make arrangements for deliveries and supplies, if you meet any of these mentioned.

Our office is closed to WALK-IN SERVICE. But CURB-SIDE SERVICE is available. All Assistance Forms available on-line and all require you complete a form. 

Stay HOME, Don’t Socialize, Double Mask, Sanitize and Stay 6 feet away. Spend no more than 15 minutes with someone who is not part of your household. SJSPT- "Help us keep our Tribal Community & Staff Safe, wash your hands, double mask up and practice social distancing."
